Neil, part of the issue is that groups such as Hesperian and the Red Cross have decided against using actual open licenses. This inhibited people other than themselves from promoting and distributing their material. It of course is there choice to use closed licenses, but it is still unfortunate as it decreases access. I guess this would be a lack of political commitment.
