I do not know much about Zambia, I know about Tanzania where a ngo(international/local) or a private entity willing to open a clinic anywhere in the country for antenatal service must first obtain the permission from Ministry of Health.
Whatever rural, remote area it is one should keep in mind that there is a Government(central/regional/distrital/local) , there are National Guidelines and Health authorities.
