Take the problem imaginatively. Satisfy at least some of the criteria (I live in a HIC and I my own blood pressure machine.

At the moment we are taking almost no-one's blood pressure properly and never will perhaps take everyone, so blood pressure properly - a similar situation to smoking, alcohol, drug taking, obesity - bit we want to do better with limited resources and a faltering global economy.

How about satisfying most of tbe blood pressure taking recommendations and put blood pressure machines in cafes, churches, restaurants - places where people sit comfortably for half an hour?
