For those interested, I have a link to a publication that provides a manual for clinicians to track as objectively as possible the effects of their interventions for any condition and a complimentary manual for patients to self-monitor the effects of their treatment.

Dead simple with case examples, suitable for translation and use in any environment.

Great training and educational instrument as well (e.g., partner gets to compare themselves with the expert clinical leader).
