I previously suggested looking at the 5 existing topics as the outcome of "interrelated and systemic causes. So that we can connect ideas at the level of the individual and clinics to the healthcare system more effectively."

The CFIR framework from the field of Implementation Science has been used in practice for more than 15 years. It has 5 domains: Intervention Characteristics, Outer Setting, Inner Setting, Characteristics of Individuals and Process (see description below).

For the purposes of informal use of mobile phones, the three domains that are probably most useful are the Outer Setting, Inner Setting and Characteristics of Individuals.

1. Intervention Characteristics

* Attributes of the intervention itself (e.g. complexity, adaptability, cost).

* Influences how stakeholders perceive and adopt the intervention.

2. Outer Setting

External influences on implementation such as:

* Patient needs and resources

* Peer pressure from other organisations

* External policies, incentives, and regulations

3. Inner Setting

Features of the implementing organization, including:

* Culture and norms

* Leadership engagement

* Communication networks

* Readiness for implementation (e.g. resources, support structures)

4. Characteristics of Individuals

Traits and behaviours of the people involved in implementation, such as:

* Knowledge and beliefs about the intervention

* Self-efficacy

* Stage of change and identity

5. Process

Steps and activities involved in implementation, including:

* Planning

* Engaging key stakeholders

* Executing the intervention

* Reflecting on and evaluating implementation

* progress
