Dear Neil, I’ve read this several times and I think you are being too harsh on yourself. In fact, if you read your piece as if it were written by one of your juniors you would agree with me. You answer your charge: inexperience, unusual condition, too busy, slow system. That counselling you should have been offered is not too late, I seriously think you should seek an appropriate older, sympathetic, skilled physician to talk to. I’m sure it doesn’t help (& probably shouldn’t) to tell you I could share much greater physician errors that I’ve witnessed in my career. Nursing ones exist too, but usually different.
