Thank you Neil for sharing that the missed diagnosis of epiglottitis was your own story. I had not realised that when I wrote my response discussing the wider issues that your story illustrated.
I think that the sharing of such stories is very helpful, particularly to younger people working in healthcare, who may feel very alone when such things happen.
These stories are also a wonderful way of teaching people about safety as one can carry out analyses and uncover all the system factors that played in to the final outcome
