Neil - in answer to your question, "A Lancet paper by Kruk et al 2018 estimated that up to 8 million people a year (of all ages) were dying due to low-quality care, but I could not find data specific to children. Does anyone have estimates on deaths due to low-quality care among children?", please see the links to a paper my colleagues and I wrote as well as a paper by Black 2016.
