Patient data, patient records, information and semantics!(2)

Dear Richard,

Please can you say more about the difference between ‘records’ and ‘data’? I understood that the main issue was to empower patients to access their personal medical records, which for me includes the notes that a health professional might jot down at each appointment. But you describe these below as ‘Professionals’ records’, which ‘belong to professionals’. Are you not pushing for patients to have access to these records also?
